Penny wise - Pound fool NHS England cuts

Ill thought out cuts to the NHS that could cost more in the long run!  See how I was snoring patient, sleep apnoeas patient and breast reduction out of necessity read on....
Penny wise pound foolish! or push towards privatisation?
I had to have a breast reduction as it was an abnormality at the menopause my boobs kept getting bigger..... to be a L and M cup given my small statue it was causing a lot of problems ... from feeling like my back muscle and skin going to tear open..... and arm ataxia from the weight of my bra strap on my shoulders......BUT the most important was that I was snoring and having sleep apnoeas....with increasing fatigue through day.  The ENT specialist  referred me to Harefield hospital where sleep studies where done and found that significant high risk sleep apneas where occurring with drops in O2 stats & bradycardia. I had to use CPAP on a night - lol going to bed looking like a fighter pilot with a mask delivering not oxygen but air under pressure if you talked with this on you sounded like Darth Vader.. 
You can see from the picture it is commonly a male problem to have sleep apnoeas - My specialist could only put the problem of sleep apnoea to the large breasts and the weight on my ribcage..... it would feel like someone sat on your chest having to force your breath to expand your ribcage on inspiration........ So the CCG finally permitted funding for my breast reduction as to continue as I was ....would have led eventually to raised BP from body in constant catecholamine effect from drops in O2 sats ... then increase risk of stroke and heart failure..... so long term I would cost more to the NHS........ I was referred to the surgeon and was happy for him to do bilateral mastectomy as so frustrated with the problems to my health.... but he wanted to preserve some breast tissue so I had bilateral mamaplexy .... reduction.... I had a tricky post op with necrosis and wound breakdown but the feeling of the weight of my chest was wonderful........ once all the wounds healed with vac therapy - we did repeat sleep studies and there was significant reduction to not need the CPAP.... so the outcome was worth all..... and I have paid in tax and NI since 1969....in my day you put a stamp on the nhs card..... so felt I was being cheated to have to fight and justify the breast reduction ..... not a frivolous Kim Khardsan whim of plastic surgery but medically necessary....

I can see the tack on these cuts - you can see that the private sector will capitalise on these operations but this means patients will be paying twice.....their contributions to the NHS and then the extra for private care...... and their need to get private medical insurance ON TOP of National insurance!! or those in poverty having problems will be denied the right to treatment and this is totally against the principles of the NHS. 
So this feels a massive push to privatisation!!!!
I feel as clinical practitioners we must as always be an advocate for our patients; and help managers see out of the narrow frame of cost cutting - to see the long term benefits of intervention when a problem is manageable. This is much better housekeeping rather than it escalate to be a more expensive medical and surgical problem down the line!

Brilliant books for Health Professionals

 I have been reconnecting with myself today as I try to find some paid work.... looking at my books on the bookshelf at home... the books that I studied with and kept updated with as a BSc RN RM in my practice have always been my friends. When I was in coronary care I found Dale Dubin's book Rapid interpretation of EKGs (US way of saying ECG) was the most useful.. Dale is like the David Attenborough of natural history and biology he has an infectious passion for his subject matter. The way he explains the subject exudes his passion for his work..... the work book is well set out to work systematically through - with even the recognition of break for cup of tea in the heavy to understand bits... lol .... love the way he got across to me the axis deviation so that as you look at the V leads and QRS complexes you can work out the area of infarction in the heart muscle..... this book is a worthy investment for any doctor or nurse or midwife to have to understand.  I include midwives as I worked in a teaching hospital St Thomas's Hospital in London and we would have women through that had pre existing heart problems that had survived to have babies.... and there were times when we had to nurse mothers in HDU High dependency unit with cardiac monitoring.... so it is good to be aware of what is normal and what is an alert on abnormality.... even in this book you find out the wave patterns for high and low potassium so electrolyte imbalances.

 This neonatal resuscitation is a work book with CD rom to learn with... a must for midwives and those working in neonatal units and paed A/E Neonatal Resus textbook and the must follow on is the stabilisation of neonates following successful resus.... for this I recommend the S.T.A.B.L.E course STABLE program S.T.A.B.L.E. stands for the six assessment and care modules in the program: Sugar, Temperature, Airway, Blood pressure, Lab work, and Emotional support

V&A shop

 
 Spotted some lovely books and ideas for Boshra in Belgium.... I really do need to get her sorted with some art materials so her creativity is allowed to blossom and can help to rebuild her life with sales of her work. There was fun things for the girls too. Under my Umbrella design for refugees

This was a good idea to engage the children to think about being a refugee and the message of love they wanted to send to refugees... See my design below as I had to sit and have a go too... The education staff for the V&A showed great interest in my Family Link Up project... Family Link Up  I hope we can meet up again and brainstorm more ideas to help keep the spotlight on refugees and the need to rebuild lives, heal the pain and gain peace and happiness and most of all hope again. 


 Some of the participants ideas then mine below in the slides.

My drawings are very elementary so forgive me on this but the idea was to use the segments of the umbrella to take you on a journey of a little girl caught in war and fleeing..... it starts with her normal life in peace with her apartment home, a normal day going to school and her pet cat...and her love of flowers.
 Then there is the devastation of airstrikes and her world destroyed as she knew it.. She is then in a refugee camp with the UNHCR.  She is with her family and others and one is shown in a wheel chair injured by the war.  The next segment shows her asleep in her tent dreaming of her home and peace an when life was good.  Then there is the flee over sea to Europe and the people overboard from the boat with their orange life jackets  on .... the is then the rescue to safe arms and it ends with peace beauty so goes full circle from Peace beauty through the horror of war and fleeing to back to peace and beauty. I have left this for the staff on the display rack in the hope that it is seen what I was trying to convey. I quite enjoyed the creativity of this and could do another with some photos and better illustrations.
